A soft tissue neck X-ray visualizes the airway and surrounding soft tissues to evaluate causes of breathing or swallowing problems, suspected foreign body, neck infection (epiglottitis, retropharyngeal abscess), trauma, or prevertebral swelling. It helps detect airway narrowing (croup, subglottic stenosis), masses, and alignment abnormalities. Clinicians use it when stridor, hoarseness, dysphagia, neck pain, or trauma raise concern.

A lateral neck X‑ray is indicated when evaluating suspected upper airway or deep‑neck pathology: foreign body ingestion/aspiration lodged in the pharynx or upper esophagus, airway obstruction from epiglottitis or croup, retropharyngeal or peritonsillar abscess (prevertebral swelling), persistent dysphagia/odynophagia, neck swelling, or after cervical trauma to assess soft‑tissue and airway compromise.

A lateral soft‑tissue neck radiograph demonstrates the airway and adjacent soft tissues from the nasopharynx to the upper trachea. It’s used to detect airway narrowing or obstruction, epiglottitis, swollen or displaced prevertebral/retropharyngeal soft tissues, deep neck abscess, laryngeal or pharyngeal masses, subglottic narrowing, and radiopaque foreign bodies; it also helps assess cervical spine–related soft‑tissue changes.

A neck X‑ray can sometimes detect large or calcified tumors, bone destruction, airway displacement, or visible masses, but it’s insensitive for most soft‑tissue neck tumors. CT, MRI, or ultrasound provide much better detection and characterization. If a tumor is suspected, more advanced imaging and biopsy are usually recommended to confirm diagnosis and plan treatment.

X-rays primarily visualize bones and joint alignment; they do not directly show most soft-tissue injuries.
