To X‑ray a thumb, remove jewelry and explain the procedure. Obtain at least three views: anteroposterior (AP), lateral and oblique. Seat the patient, immobilize the hand with a sponge or tape, and position the thumb on the receptor so the joint of interest is centered. Collimate to the thumb, apply gonadal shielding, and instruct the patient to remain still during the exposure. Review images for adequate coverage.

An AP (anteroposterior) X‑ray projects the beam front-to-back and is used when patients can’t stand or need portable imaging. It’s commonly performed for chest (ICU/trauma), abdomen, pelvis and limb films to evaluate fractures, joint alignment, lung fields, organ size/position and foreign bodies. AP views can magnify anterior structures, so findings sometimes need confirmatory PA or lateral images.

Bett's view of the thumb is a specific radiographic oblique projection used to visualise the first metacarpal base and carpometacarpal joint. It helps detect fractures (notably Bennett’s fracture) and joint subluxation by rotating the hand so the thumb is obliquely projected onto the image receptor, separating overlapping bones for clearer assessment.

Get a thumb X‑ray after trauma if there is visible deformity, severe swelling or bruising, an open wound, inability to move or use the thumb (or to grip), focal bone tenderness at the base of the thumb or anatomical snuffbox, pain with axial compression of the thumb, suspected dislocation, or ligament injury (e.g., skier’s thumb) with instability. Also image persistent pain or numbness after injury.

Hand AP position: The patient’s forearm is supinated with the palm facing upward and the hand flat on the image receptor. Fingers are extended and slightly separated; the wrist and distal forearm are included. The central X‑ray beam is perpendicular to the third metacarpophalangeal joint. Immobilize and use markers to minimize motion and ensure correct orientation.

Yes. A thumb X‑ray can detect many forms of arthritis by showing joint space narrowing, bone spurs (osteophytes), subchondral sclerosis, cysts, and erosions in established disease. X‑rays are useful for diagnosing osteoarthritis and assessing severity, but early inflammatory changes or soft‑tissue involvement may not appear; clinicians may use ultrasound, MRI, blood tests, and clinical exam for earlier or more detailed assessment.
