X Ray Both Wrists Oblique View

An oblique wrist X-ray provides a rotated view that helps reveal fractures, dislocations, and joint misalignments not clearly seen on straight AP or lateral views. It improves visualization of carpal bones (especially the scaphoid and lunate), the distal radius and ulna, and radiocarpal joint spaces, aiding diagnosis of occult fractures, ligament injuries, arthritis, and bone lesions, and guiding further imaging or treatment.

An oblique view in X‑ray is a deliberately angled projection between the frontal (AP/PA) and lateral planes that images structures not well seen on standard views. By rotating the patient or the x‑ray beam, it reduces overlapping of bones and soft tissues, improving visualization of fractures, joint spaces, and subtle pathology. Oblique views complement other projections for a more complete assessment.

The two standard wrist X‑ray views are the posteroanterior (PA) and lateral. PA view (hand palm down) shows carpal bones, joint spaces and distal radius/ulna relationships. Lateral view (hand on its side) demonstrates sagittal alignment, dorsal/volar displacement and carpal stacking. They are often supplemented by an oblique or scaphoid-specific view when needed.

Position the hand in a 45° oblique: seat the patient, rest the forearm and wrist on the cassette, pronate the hand and rotate it laterally about 45° so the metacarpals and digits lie oblique to the receptor. Fingers extended and slightly separated; thumb abducted. Center at the MCP joints and direct the central ray perpendicular to the 3rd MCP. Immobilize to prevent motion.
