The scaphoid view is a special wrist X‑ray projection designed to visualize the scaphoid bone and its waist. It’s taken with the wrist in ulnar deviation (often with slight pronation) and the X‑ray beam angled about 20–30° toward the elbow. This projection reduces bone overlap, improving detection of scaphoid fractures and guiding early diagnosis and management.

The best initial wrist X‑rays are posteroanterior (PA) and lateral views, with an oblique view commonly added. For suspected scaphoid fractures, a dedicated scaphoid (PA with ulnar deviation) projection improves visualization. If radiographs are normal but clinical suspicion persists, consider advanced imaging (CT or MRI). Ensure correct positioning and multiple views for accurate assessment.

The PA scaphoid (ulnar deviation) projection best visualizes the scaphoid. The wrist is placed in ulnar deviation while the central X‑ray beam is angled proximally about 10–20° toward the elbow; this reduces foreshortening and profiles the scaphoid. Additional views (lateral and oblique) or repeat scaphoid views may be obtained if a fracture is suspected.

The standard wrist X‑ray series includes three views: a posteroanterior (PA) or AP projection, a lateral projection, and an oblique projection (usually 45°). These views assess bone alignment, joint spaces, and common fractures such as scaphoid injuries; additional dedicated scaphoid or specialized views may be added if clinically indicated. They provide complementary angles for accurate diagnosis.

A scaphoid X‑ray is indicated at any age when a scaphoid fracture is suspected—typically after a fall onto an outstretched hand with anatomical snuffbox tenderness. Scaphoid fractures are most common in adolescents and young adults (around 15–30 years). In children and very early injuries, initial X‑rays may be normal; persistent suspicion merits repeat films, immobilization, or advanced imaging (MRI/CT).

The scaphoid is a small carpal bone on the thumb (radial) side of the wrist that stabilizes and links the proximal and distal carpal rows. It transmits forces from the hand to the forearm, allows smooth wrist motion, and maintains joint congruity.
