Typhoid Test

Typhoid is diagnosed mainly by culture: blood culture early in illness and bone marrow culture (most sensitive). Stool and urine cultures are useful later. Serologic Widal testing has limited accuracy; rapid antibody tests (Typhidot, Tubex) give quicker but variable results. PCR/molecular assays detect Salmonella DNA and increase sensitivity. Clinical features and exposure history guide test selection.

If typhoid test is positive, you’ll need prompt antibiotic treatment guided by susceptibility testing, plus fluids, fever control, and monitoring. Severe cases may require hospitalization for IV antibiotics and management of complications such as intestinal perforation, hemorrhage, or sepsis. Follow-up stool cultures confirm eradication; some people become chronic carriers and need additional treatment. Avoid preparing food until cleared.

On CBC, typhoid often shows low or normal total white cell count (leukopenia) with relative lymphocytosis or neutropenia, eosinopenia, mild anemia, and sometimes thrombocytopenia. CBC findings are supportive but not diagnostic—confirm with blood, stool or bone marrow culture or specific serology and correlate with clinical signs.

Normal typhoid results: blood culture — no Salmonella growth (negative). Widal test — baseline (non‑reactive) titres are generally under 1:80 for O and H antibodies (some labs use <1:160); higher titres or a fourfold rise suggest infection. Rapid serology (Typhidot) — negative IgM/IgG is normal; positive IgM indicates recent infection. Clinical correlation and repeat testing are important.

Yes. CRP is usually elevated in typhoid fever as part of the acute‑phase inflammatory response; levels are often moderately raised and fall with effective treatment. However, CRP is non‑specific and cannot confirm typhoid alone, so results must be interpreted with clinical findings and microbiological tests (blood cultures/serology). Very high CRP suggests additional or alternative bacterial infection.

A positive Widal test indicates antibodies (agglutinins) against Salmonella typhi or paratyphi O and H antigens, suggesting a current or recent typhoid/paratyphoid infection. It can be influenced by prior vaccination, past infection, or cross-reacting organisms, producing false positives. Clinical correlation and confirmatory tests — notably blood culture or repeat serology — are needed before starting treatment.
