Thallium

Thallium is a highly toxic heavy metal that rapidly distributes in the body, disrupting potassium-dependent processes and mitochondrial function. Acute poisoning causes nausea, abdominal pain, vomiting, diarrhea, confusion, and painful peripheral neuropathy; characteristic hair loss appears days to weeks later. Severe exposure can produce cardiac arrhythmias, coma, and multi-organ failure. Treatment includes supportive care, potassium supplementation and Prussian blue to enhance elimination.

Thallium can accumulate in certain foods, especially root vegetables (potatoes, carrots), leafy greens, cereals and grains, legumes and nuts, and some seafood (particularly shellfish and certain fish). Organ meats and products grown in contaminated soil or irrigated with polluted water may also contain elevated thallium. Levels vary by geography and industrial contamination, so monitoring and sourcing matter.

Thallium poisoning can be very painful. Early symptoms include severe abdominal cramping, nausea, and vomiting, followed in days by painful peripheral neuropathy with burning, tingling, numbness and muscle pain. Neuropathic pain and weakness can be intense and persistent; hair loss and systemic effects may follow. Severity depends on dose and time to treatment; prompt medical care reduces complications.

Thallium is a naturally occurring heavy metal present in trace amounts in Earth’s crust, often associated with minerals and sulfide ores (e.g., potassium, zinc, copper). It can be released by volcanic activity, coal burning, and mining/smelting. Industrial uses (electronics, glass, optics) and former rodenticides can cause contamination; traces may be found in soil, water, food crops, and industrial effluents.

Thallium metal and its common salts are essentially odorless and have no distinctive smell. Because many thallium compounds are colorless and tasteless as well, accidental exposure or contamination may go unnoticed—one reason it was historically used as an undetectable poison. Symptoms include hair loss, neuropathy, and severe gastrointestinal distress, so seek urgent medical attention if exposure is suspected.

Thallium is not used as a treatment in hospitals because thallium metal and salts are highly toxic. However, a radioactive isotope, thallium-201, is used in nuclear medicine for myocardial perfusion imaging (cardiac stress tests). Its use has declined as safer radiotracers such as technetium-99m are preferred, though thallium-201 remains available for some diagnostic cases.
