Scrub Typhus IgM

IgM positive in scrub typhus indicates a recent or acute infection with Orientia tsutsugamushi. IgM antibodies typically appear about one week after symptoms begin and peak in the first few weeks, so a positive result supports diagnosis when paired with compatible symptoms and exposure. False positives and cross‑reactivity can occur, so clinicians may confirm with repeat testing, PCR or paired sera and start appropriate antibiotics.

If scrub typhus is confirmed, begin prompt antibiotic treatment—doxycycline (adult 100 mg twice daily for 7–14 days) or azithromycin for pregnancy/young children. Hospitalize severe cases for IV antibiotics, fluids, fever control and respiratory/organ support. Monitor vitals, liver and kidney function and platelets for complications (ARDS, encephalitis, renal failure). Reassess within 48–72 hours and adjust therapy if no improvement.
