PERIANAL ULTRASOUND

Perianal ultrasound is used to assess the anal sphincters for tears or scarring (commonly after childbirth or trauma), detect and delineate perianal abscesses, fistula tracts and fluid collections, evaluate masses or tumors, and guide surgical planning and treatment. It helps identify causes of fecal incontinence, occult sepsis and monitor postoperative healing, offering a quick, noninvasive local evaluation.

Perineal ultrasound is a noninvasive imaging technique that uses high-frequency sound waves to visualize the pelvic floor and lower genital and urinary tracts through the perineum. A transducer placed on the skin between the vagina and anus captures real-time images to assess pelvic floor muscles, childbirth-related tears, prolapse, incontinence, and surgical outcomes. It’s painless, radiation-free, and useful for diagnosis and monitoring.

Yes — ultrasound can detect many superficial and perianal fistulas; specialized techniques (endoanal/endorectal, transperineal, or transvaginal ultrasound) often identify fistula tracts and associated abscesses. However, ultrasound is less sensitive for deep, complex, or recurrent fistulas, and MRI is usually preferred for detailed mapping. Ultrasound remains a useful, accessible first-line tool and for procedural guidance.

Ultrasound is not routinely used to diagnose hemorrhoids (piles). Clinical examination and anoscopy/proctoscopy are primary. Specialized endoanal or transperineal ultrasound can sometimes detect thrombosed external hemorrhoids, perianal abscesses, or sphincter defects, but has limited sensitivity for uncomplicated internal hemorrhoids. If symptoms are atypical or suggest more serious disease, doctors may order imaging (ultrasound or MRI) for further evaluation.

Perianal treatment addresses disorders of the skin and tissues around the anus—haemorrhoids, fissures, abscesses, itching and infections. Management includes hygiene and sitz baths, topical agents (steroid or anesthetic creams, barrier ointments, topical antibiotics), oral analgesics, stool softeners and diet changes. For infection or complicated disease, systemic antibiotics or minor procedures and surgery may be required; follow-up prevents recurrence.

Magnetic resonance imaging (pelvic MRI with contrast) is the best scan for suspected perianal abscess. It accurately defines abscess size, location, relation to the sphincter complex and any associated fistula, guiding surgical planning. Endoanal ultrasound is a useful alternative for superficial lesions; CT may be used in emergencies or when MRI is unavailable.
