Most cervical spine MRIs are performed without contrast; plain MRI adequately evaluates discs, spinal cord, stenosis, and nerve compression. Gadolinium contrast is reserved for specific indications—suspected tumor, infection, inflammation, or to distinguish scar tissue from recurrent disc after surgery. The radiologist decides based on clinical history and imaging needs; contrast is avoided or used cautiously in patients with severe renal impairment.

Use is avoided or adjusted in severe renal impairment because of nephrogenic systemic fibrosis risk, and it’s used cautiously in pregnancy or with a prior contrast allergy.

A dorsal spine MRI is a magnetic resonance imaging scan of the thoracic (middle) spinal column that produces detailed images of vertebrae, intervertebral discs, spinal cord and nerve roots. It’s used to evaluate pain, trauma, infection, tumors, inflammation or degenerative disease. The non‑invasive exam takes about 20–45 minutes, may use contrast, and requires removal of metal; some implanted devices are contraindicated.

A cervical spine MRI with contrast typically takes about 30–60 minutes. The actual scanning portion is usually 20–40 minutes, plus roughly 10–20 minutes for IV placement, contrast injection and patient positioning. Total time varies with the number of imaging sequences, need for delayed images, patient cooperation and facility scheduling, so allow extra time at the appointment.

Contrast is not mandatory for MRI. Gadolinium agents are used selectively to better detect and characterize tumors, infections, inflammation, and vascular lesions. Whether contrast is needed depends on the clinical question, kidney function, pregnancy status, and allergy history. Advanced noncontrast sequences (diffusion, FLAIR, time-of-flight MR angiography) often provide sufficient diagnostic information without contrast.
