MR angiography can be done without contrast using techniques such as time‑of‑flight (TOF) and phase‑contrast MRA to visualize blood flow, especially in intracranial and some extracranial vessels. Non‑contrast MRA is preferred when gadolinium is contraindicated (e.g., renal impairment). Contrast‑enhanced MRA, however, often provides faster exams and better vessel detail; non‑contrast studies can show lower resolution and flow artifacts.

A neck MRI without contrast provides detailed images of soft tissues and the cervical spine: vertebrae, intervertebral discs, spinal cord, nerve roots, ligaments, muscles, and many masses such as cysts, infections, inflammation and degenerative changes (herniated discs, spinal stenosis). It detects abscesses and most structural problems but has limited ability to characterize vascular abnormalities or distinguish certain tumors and subtle inflammatory lesions that often need contrast.

Contrast (gadolinium) is not routinely required for most neck MRI studies—degenerative disease, trauma, and routine spinal assessments are usually well visualized without it. Contrast is helpful when evaluating tumors, infection/abscess, postoperative scar versus recurrence, vascular lesions, or unclear soft‑tissue masses. Use is individualized by the clinical question and patient factors; gadolinium is generally safe but avoided or used cautiously with severe renal impairment.

MRA of the neck can be performed either with intravenous gadolinium contrast or without it. Noncontrast methods such as time of flight and phase contrast can show arteries and avoid contrast risks. These estimate presence and severity of blockages; CT coronary angiography shows detailed anatomy. Definitive treatment or complex diagnosis may still require invasive angiography—choice depends on symptoms and risk.
