MRI BRAIN PERFUSION SCAN

An MRI brain perfusion scan measures blood flow and circulation dynamics within brain tissue to assess oxygen and nutrient delivery. Using contrast-enhanced techniques or noncontrast arterial spin labeling, it maps cerebral blood volume, flow and mean transit time. Clinically it helps detect acute ischemia, stroke penumbra, tumor grade, and vascular disorders. The test is done during an MRI session and may require intravenous contrast.

An MRI with perfusion is a specialized scan that measures blood flow through tissues, usually using contrast (gadolinium) or non‑contrast arterial spin labeling. It generates maps of blood volume, blood flow and mean transit time to assess tissue viability and vascular supply. Clinically it helps evaluate stroke, tumour blood supply, and other conditions by identifying regions with reduced or abnormal perfusion.

Duration varies by technique. CT perfusion scans take only minutes for imaging (often 1–10 minutes), with a total appointment around 15–30 minutes. MR perfusion appointments typically last 30–60 minutes, with the perfusion sequence itself taking a few minutes. Nuclear medicine (SPECT) perfusion studies are longer—usually 1–3 hours total, allowing for tracer injection, uptake, and scanning.

Perfusion MRI can use contrast agents but not always. Contrast-based techniques—dynamic susceptibility contrast (DSC) and dynamic contrast‑enhanced (DCE) MRI—use gadolinium to map blood volume and permeability, useful in stroke and tumor evaluation. A non‑contrast alternative, arterial spin labeling (ASL), labels blood magnetically to measure cerebral blood flow. Choice depends on clinical question, patient kidney function, and required sensitivity.

A perfusion scan evaluates blood flow (perfusion) to organs by tracking a small radiotracer with a gamma camera. It identifies areas with reduced or absent blood supply—commonly used to detect pulmonary embolism, assess regional lung function before surgery, and evaluate cardiac or cerebral ischemia. It helps guide diagnosis, treatment planning, and monitoring response to therapy. The test is minimally invasive.

MRI can detect many kinds of brain damage — structural changes from stroke, bleeding, tumors, demyelination, swelling, scarring or atrophy. Diffusion-weighted MRI is especially sensitive for acute ischemic injury. However, MRI may not reveal microscopic, metabolic or purely functional injuries (mild traumatic axonal injury or early changes), so additional tests (CT, PET, EEG, clinical assessment) may be needed for full evaluation.
