Malarial Antigen

A malaria antigen is a molecule—usually a parasite protein or glycoprotein—expressed by Plasmodium stages that elicits an immune response. Common examples include circumsporozoite and merozoite surface proteins and variant surface antigens (e.g., PfEMP1) on infected red blood cells. These antigens are targets for diagnostic tests and vaccine development; their presence or antibodies indicate infection or prior exposure.

The main malaria antigens used for detection and immune responses include histidine‑rich protein 2 (HRP2) — specific to Plasmodium falciparum — parasite lactate dehydrogenase (pLDH), and aldolase, which are pan‑malarial. In infected red blood cells, parasite proteins such as PfEMP1 are expressed on the surface and mediate cytoadherence and antigenic variation. These antigens are targeted by rapid diagnostic tests.

Malaria P. vivax and P. falciparum antigens are parasite proteins found in blood used for diagnosis and immune recognition. Key examples include species-specific pLDH isoforms and aldolase (pan-species), and P. falciparum histidine-rich protein 2 (HRP2). These antigens are targets of rapid diagnostic tests and serology. Some HRP2 gene deletions can cause false negatives, so combined antigen detection improves accuracy.

Malaria antigen tests vary: HRP2-based tests can remain positive for weeks after successful treatment—commonly 2–6 weeks and sometimes longer—because antigen fragments persist. By contrast, pLDH-based tests usually turn negative within a few days to about a week once parasites are cleared. A lingering positive HRP2 result doesn’t always indicate active infection; clinical correlation or microscopy/PCR may be needed.

Bulk procurement prices for RDT kits are often about $0.20–$5 per test; retail or private clinic charges commonly range $10–$50 (sometimes up to $100) including service fees. Public health systems or national labs may provide testing free. Final cost depends on country, provider, test brand and whether lab or consultation fees apply.
