Magnesium in a 24‑hour urine test measures the total amount of magnesium excreted over a full day. It helps evaluate body magnesium balance, absorption, and kidney handling. Low urinary magnesium can indicate deficiency or poor intake/absorption; high values may reflect excessive intake, supplements, or renal wasting. Results are interpreted with blood magnesium levels and clinical context to assess status and causes.

Magnesium is filtered and regulated by the kidneys, so higher oral intake increases urinary magnesium excretion. Normal amounts usually don’t noticeably change urine, but certain forms (e.g., magnesium citrate) raise urinary citrate, which can reduce kidney stone risk. Very high doses cause gastrointestinal upset and, in kidney impairment, can lead to magnesium buildup and reduced urine output with potentially serious effects.

Magnesium absorbed from food or supplements begins appearing in urine within a few hours; with normal kidney function most excess magnesium is eliminated within 24–48 hours. Blood and urinary levels adjust quickly, but body magnesium stores in bone and tissues change over days to weeks. Impaired kidney function slows excretion and can cause buildup, so dosing should be adjusted if renal disease is present.

High urinary magnesium (hypermagnesuria) generally indicates excess intake or increased renal loss of magnesium. Causes include recent magnesium supplements or laxatives, use of diuretics, osmotic diuresis (e.g., uncontrolled diabetes), or tubular disorders that impair renal reabsorption. It may coexist with low blood magnesium from renal wasting. Evaluation by a clinician with blood tests and medication review is recommended.

Normal urinary magnesium excretion (24‑hour collection) is roughly 3–5 mmol/day (≈73–122 mg/day). Spot urine values are interpreted using lab-specific reference ranges or a magnesium/creatinine ratio and vary with dietary intake, renal function, and certain medications.
