LUPUS ANTICOAGULANT IGM TEST

A positive lupus anticoagulant test indicates antiphospholipid antibodies that paradoxically prolong clotting tests yet increase risk of abnormal blood clots and pregnancy complications (recurrent miscarriage). It can be transient after infection or medication, so repeat testing at least 12 weeks is needed to confirm antiphospholipid syndrome. Management depends on clot history and may include anticoagulation and specialist follow‑up.

Lupus anticoagulant (an antiphospholipid antibody) can be present during pregnancy and is linked to antiphospholipid syndrome. It raises risks of miscarriage, stillbirth, preeclampsia, fetal growth restriction, preterm birth and maternal thrombosis. Testing is indicated after thrombosis or pregnancy loss and should be confirmed 12 weeks apart. Management typically includes low‑dose aspirin with low‑molecular‑weight heparin and close obstetric monitoring to reduce complications.

The presence of a lupus anticoagulant is confirmed with specialized coagulation assays—chiefly the dilute Russell viper venom test (dRVVT) and an aPTT-based LA panel. Diagnosis requires screening tests, mixing studies, and confirmatory phospholipid-neutralization procedures. Positive results must be persistent, with repeat testing at least 12 weeks later to meet diagnostic criteria for antiphospholipid antibody–related disorders.

A positive lupus test means laboratory evidence that the immune system is producing autoantibodies linked to systemic lupus erythematosus. Typical findings include a positive ANA and more specific antibodies such as anti–double‑stranded DNA or anti‑Smith; low complement or antiphospholipid antibodies may also be present. A positive result raises suspicion for lupus but requires clinical correlation and further evaluation by a healthcare professional.

Early warning signs of lupus include persistent fatigue, unexplained fever, joint pain or swelling, and a distinctive butterfly-shaped facial rash. Other early symptoms: photosensitivity (worsening rash after sun), hair loss, mouth or nasal ulcers, Raynaud’s (cold-triggered finger/toe color changes), chest pain or shortness of breath, swollen lymph nodes, and occasional cognitive fog or memory problems. If lupus is left untreated, ongoing inflammation can cause progressive, potentially irreversible damage to organs—especially kidneys (lupus nephritis), heart and lungs, brain and nervous system, and joints. Risks include kidney failure, cardiovascular disease, severe infections, seizures, stroke, chronic pain, scarring, increased morbidity and mortality, and pregnancy complications.
