IA 2 ANTIBODY

IA-2 antibodies are autoantibodies directed against IA-2, a protein in insulin-producing pancreatic beta cells. Their presence indicates autoimmune targeting of beta cells and is a common marker in type 1 diabetes, often appearing with other autoantibodies (GAD, insulin). Detection helps diagnose and predict progression to type 1 diabetes in at‑risk individuals.

A “normal” (negative) IA‑2 antibody result is any value below the laboratory’s specific reference cutoff. Cutoffs vary by assay and lab but are commonly around <0.7–1.0 U/mL (or an equivalent assay index); values above the lab’s threshold are reported as positive and suggest autoimmune activity. Always compare your result with the lab’s reference range and discuss abnormal findings with your clinician.

IA-2 antibodies often appear before or at type 1 diabetes diagnosis and commonly persist for months to years, but their levels usually decline over time and can become undetectable in some people. Loss of detectable antibodies doesn’t reverse established beta‑cell damage or change the diagnosis; it simply reflects waning immune markers and varies between individuals.

IA-2 stands for Islet Antigen‑2, a transmembrane protein (also called ICA512) located in insulin‑producing pancreatic beta‑cell secretory granules. It resembles a protein tyrosine phosphatase and is a major autoantigen in type 1 diabetes. Autoantibodies to IA‑2 are used clinically as biomarkers to detect autoimmune beta‑cell destruction and to assess risk or progression of type 1 diabetes.

A positive antibody test usually means past infection or vaccination and suggests some immune response, but doesn’t guarantee complete or long-term protection. It can’t precisely date the infection or confirm current infectiousness. Continue precautions if symptomatic or exposed, and follow vaccination guidance. Type 2 antibodies are typically IgG or IgM that bind antigens on cells or the extracellular matrix, causing antibody‑mediated (Type II) hypersensitivity. Examples include autoimmune hemolytic anemia, Goodpasture syndrome, and antibody‑mediated forms of myasthenia gravis.
