A folate (folic acid) serum blood test measures the amount of folate circulating in the blood to detect deficiency or excess. It helps evaluate unexplained anemia, assess nutritional status in pregnancy, monitor malabsorption, alcoholism, or certain medications, and guide supplementation. Serum folate reflects recent intake; red blood cell folate better indicates long-term folate stores.

Why is my serum folate level low?plus

Low serum folate can result from inadequate dietary intake (poor diet, alcoholism), increased needs (pregnancy, rapid growth, hemolysis), malabsorption (celiac disease, small bowel surgery, chronic diarrhea), certain medications (methotrexate, anticonvulsants, trimethoprim, sulfasalazine), and increased loss (hemodialysis). Lab factors matter too: serum folate reflects recent intake, while red blood cell folate indicates long-term status.

What if your folate serum is high?plus

High serum folate usually reflects recent high dietary or supplement intake and is often harmless. However, it can mask vitamin B12 deficiency (risking neurological damage), and very high folic‑acid levels have been linked to adverse effects in specific situations. Review supplements and fortified foods, have your B12 and liver tests checked, and discuss findings and any medication interactions with your clinician.

Which foods are high in folate?plus

Foods high in folate include dark leafy greens (spinach, kale, collards), legumes (lentils, chickpeas, black beans), liver, asparagus, Brussels sprouts, avocados, citrus fruits (oranges, grapefruit), fortified cereals and breads, nuts and seeds (peanuts, sunflower seeds), and beets. Including a variety of these in meals helps meet daily folate needs, especially during pregnancy.
