Entamoeba Histolytica Detection

Detect Entamoeba histolytica by examining multiple fresh stool samples microscopically for trophozoites or cysts, and using stool antigen tests or PCR to distinguish E. histolytica from nonpathogenic Entamoeba species. For suspected extraintestinal disease, serology (antibody testing) and imaging (ultrasound/CT) of the liver help detect abscesses. Colonoscopy with biopsy and histology may confirm invasive colitis.

Diagnosis of amoebiasis is usually by stool examination for Entamoeba histolytica cysts or trophozoites, stool antigen tests or PCR to distinguish E. histolytica from nonpathogenic species, and serology (antibody tests) for suspected liver abscess. For extraintestinal disease, imaging (ultrasound/CT) of the liver with aspiration and molecular testing may be used.

The serology test for Entamoeba histolytica detects circulating anti‑E. histolytica antibodies (usually IgG) using ELISA or indirect hemagglutination. It’s sensitive for invasive disease (e.g., amebic liver abscess) and helps when stool tests are negative, but antibodies can persist after past infection and cannot reliably distinguish current intestinal colonization from prior exposure. Interpret with clinical and imaging findings.

The rapid test for Entamoeba histolytica is a stool antigen detection assay—typically an immunochromatographic (lateral-flow) or rapid ELISA—targeting E. histolytica–specific antigens (eg, Gal/GalNAc lectin). It distinguishes pathogenic E. histolytica from nonpathogenic E. dispar, yields results in minutes to a few hours, and is more accurate than microscopy; PCR or serology can confirm diagnosis.

Entamoeba histolytica ranges from asymptomatic carriage to severe invasive disease. It commonly causes bloody diarrhea and colitis; in some patients it can invade the liver causing abscesses, and rarely bowel perforation, sepsis, or death. Risk increases with poor sanitation and immunosuppression. Prompt diagnosis and combined systemic plus luminal antiparasitic treatment usually cures infection, but complications can be serious if untreated.

Early symptoms of amoebic infection often include abdominal cramps, cramping pain, loose stools that may contain blood or mucus, frequent bowel movements, and urgency. Other initial signs can be fever, nausea, vomiting, fatigue, and abdominal tenderness or bloating. In some cases, tenesmus (feeling of incomplete evacuation) appears. If symptoms are severe, persistent, or include high fever or right‑upper‑quadrant pain, seek medical care.
