Elastography measures tissue stiffness to detect scarring, fibrosis or tumors. Commonly used for liver fibrosis assessment, it also helps evaluate breast, thyroid, prostate and musculoskeletal lesions. Performed with ultrasound or MRI, it’s noninvasive and aids diagnosis, staging and monitoring of disease, guiding treatment decisions and reducing need for biopsy in many cases.

Elastography provides noninvasive, quantitative assessment of tissue stiffness, improving detection and staging of liver fibrosis and characterizing focal lesions in breast, thyroid, and prostate. It helps guide biopsy decisions, monitor treatment response, and reduce unnecessary invasive procedures. The technique is fast, safe, repeatable, and enhances diagnostic confidence by adding objective stiffness measurements to conventional ultrasound or MRI, aiding earlier and more accurate clinical management.

Elastography is usually quick — most standalone liver stiffness scans take about 5–15 minutes. When elastography is added to a full ultrasound or requires multiple measurements, the appointment can last 20–30 minutes. Total clinic time may be longer for check‑in and prep; fasting or breathing instructions might be given. The procedure is noninvasive and generally completed within a single short visit.

There are two principal types of elastography: strain elastography (qualitative, comparing tissue deformation) and shear-wave elastography (quantitative). Shear-wave techniques include transient elastography (FibroScan), point shear-wave, and 2D/3D shear-wave imaging; magnetic resonance elastography is a separate modality. Choice depends on the clinical indication and organ examined.

Costs vary widely. Transient/ultrasound elastography (FibroScan) typically costs about ₹2,000–8,000 in India and roughly $100–$500 privately in other countries. MR elastography is considerably pricier—commonly several hundred to a few thousand dollars or equivalent. Public systems like the NHS cover clinically indicated tests free of charge. Exact prices depend on country, facility, and insurance.

A normal liver elastography (FibroScan) score is generally below about 5.5–6.0 kPa, corresponding to little or no fibrosis (F0–F1). Values around 5.5–7.0 kPa may be borderline; 7–9 kPa suggests significant fibrosis, and >12–14 kPa indicates cirrhosis. Thresholds vary by device, operator, and clinical context, so interpret results with clinical correlation.
