CT SCAN TEMPORAL BONE CONTRAST

Temporal bone CT is usually performed without intravenous contrast because high-resolution, bone-window images best evaluate osseous detail, fractures, the ossicular chain, and cholesteatoma. Contrast-enhanced CT or CT angiography is reserved for specific cases—suspected neoplasm, vascular lesion, or active infection with possible intracranial/soft-tissue extension—to assess soft-tissue enhancement or vascularity. Clinicians decide based on clinical indications.

A temporal bone CT scan is a high-resolution X‑ray imaging test that shows the delicate bones and air spaces of the ear and surrounding skull. It helps diagnose hearing loss, chronic ear infections, cholesteatoma, fractures, tumors, and congenital ear anomalies. The outpatient procedure takes minutes, may use contrast sometimes, and provides detailed bone anatomy while involving a small amount of ionizing radiation.

High-resolution CT is highly accurate for temporal bone bony pathology—fractures, ossicular disruption, mastoid disease, and otosclerotic changes—because it provides excellent spatial resolution of fine bone detail. It is less reliable for purely soft-tissue lesions (e.g., cholesteatoma or tumors), where MRI (including diffusion-weighted sequences) is superior. CT is often first-line for evaluation and surgical planning.

For a temporal bone CT, bring your referral/ID and any prior imaging. Remove jewelry, hairpins, glasses and hearing aids. Tell staff if you’re pregnant, breastfeeding, have kidney issues, diabetes (metformin) or contrast allergies—some scans use IV contrast and may need fasting or specific instructions. Continue usual meds unless told otherwise, arrive early, and follow the technologist’s directions to lie very still during the scan.

A dedicated temporal-bone CT for adults usually yields an effective dose around 0.03–0.3 mSv depending on scanner settings and protocol; children require size‑adjusted lower doses. Modern low‑dose or cone‑beam protocols can reduce exposure further. This is much less than a standard head CT (about 2 mSv) and a small fraction of annual background radiation. Ask your radiology team for the exact estimate.

A non-contrast head CT is fast and excellent for detecting acute brain hemorrhage, skull fractures, acute hydrocephalus, and dense lesions or calcifications. It’s the first-line scan to rule out bleeding before thrombolysis for suspected stroke and is used when contrast is contraindicated (eg, allergy or renal impairment). It’s widely available and provides immediate information for emergency care and surgical planning.
