It helps diagnose sacroiliitis, degenerative changes, fractures, infection, or ankylosing spondylitis and assesses joint spacing and bone erosion. The scan is quick, noninvasive, may use contrast occasionally, and provides superior bone detail compared with plain X‑rays to guide diagnosis and treatment planning.

On a CT scan, "SI" usually refers to the sacroiliac (SI) joint—the paired joints that connect the sacrum to the ilium of the pelvis. CT shows detailed bone anatomy to detect fractures, degenerative changes, sacroiliitis, infection or ankylosing spondylitis. Findings help guide diagnosis and treatment for pelvic pain, trauma, or inflammatory conditions, correlated with symptoms and other tests.

Best treatment is a stepwise approach: start with conservative care—activity modification, NSAIDs, heat/ice, pelvic‑stabilizing physical therapy and a pelvic belt—plus manual therapy and targeted home exercises. If pain persists, image‑guided corticosteroid injections or prolotherapy can help; radiofrequency ablation may provide longer relief. Sacroiliac fusion is reserved for refractory, severe cases after conservative and interventional options fail.

Sacroiliac joint pain is discomfort from the sacroiliac joint where the spine’s sacrum meets the pelvis’ ilium. MRI is generally better for sacroiliac (SI) joint inflammation, marrow edema and soft-tissue changes—so it's preferred for suspected early sacroiliitis or unexplained pain. CT provides superior bone detail, showing fractures, erosions, sclerosis and chronic ankylosis, and is useful for surgical planning. Choose MRI when inflammation is suspected; choose CT for detailed bony assessment. CT uses radiation; MRI does not.
