BCR-ABL1 Quantitative by RT PCR

“BCR‑ABL1 positive” means cells carry the BCR‑ABL1 fusion gene from the Philadelphia chromosome (t(9;22)), producing a constitutively active tyrosine kinase that drives uncontrolled white‑cell growth. It’s diagnostic for most chronic myeloid leukemia and some acute lymphoblastic leukemias, detected by PCR/FISH/cytogenetics, and predicts responsiveness to targeted tyrosine‑kinase inhibitors while requiring molecular monitoring of treatment.

The BCR-ABL quantitative test measures the amount of BCR-ABL fusion gene transcript in blood or bone marrow using real-time PCR. It's used to diagnose and monitor chronic myeloid leukemia, assess response to tyrosine kinase inhibitor therapy, detect minimal residual disease, guide treatment changes, and identify molecular relapse. Results are reported on an international scale to track trends over time.

RT-PCR for BCR-ABL is a laboratory test that detects and quantifies the BCR-ABL fusion gene transcript produced by the Philadelphia chromosome in leukemias (commonly CML). Reverse transcription converts RNA to DNA, then PCR amplifies and measures transcript levels (qRT-PCR), allowing sensitive detection of disease, monitoring treatment response and minimal residual disease. Results are reported on an international scale to guide therapy decisions.

A PCR test for leukemia uses polymerase chain reaction to detect and quantify specific genetic abnormalities or fusion transcripts in leukemia cells (from blood or bone marrow). It amplifies DNA or RNA to identify mutations, fusion genes and measure minimal residual disease with high sensitivity. PCR aids diagnosis, classification, prognosis, monitoring treatment response and early relapse detection by tracking molecular disease burden over time.

Treat BCR‑ABL1–positive patients primarily with BCR‑ABL tyrosine kinase inhibitors (imatinib, dasatinib, nilotinib, bosutinib; ponatinib for T315I/resistance). For Ph+ ALL, combine a TKI with chemotherapy. Monitor response with regular quantitative BCR‑ABL PCR and adjust therapy or consider allogeneic stem‑cell transplant for inadequate response or advanced disease. Provide supportive care and manage drug interactions, side effects, and adherence.

Normal BCR‑ABL1 in healthy people is undetectable (0% on the International Scale). In CML monitoring, response targets are: major molecular response (MMR) ≤0.1% IS (≥3‑log reduction), deep responses MR4 ≤0.01% IS (≥4‑log), and MR4.5 ≤0.0032% IS (≥4.5‑log). Assay sensitivity varies by lab, so discuss results with your treating clinician.
