A positive anti-HBc total result means antibodies to the hepatitis B core antigen are present (IgM and/or IgG), indicating past exposure or current infection with hepatitis B. It is not produced by vaccination. Further testing—HBsAg, anti-HBs and sometimes HBV DNA—is needed to distinguish acute, chronic, resolved infection, or an isolated core-antibody result and to guide management.
